Dr. Miladinovic cannot issue any notes for Covid Vaccine exemption unless documentary proof is provided showing patient has had an anaphylaxis reaction to a vaccine or:

  1. Patient had a severe reaction to a previous dose of mRNA vaccine

  2. Patient had a case of myocarditis following a vaccine
